Cushing’s Understanding Support & Help Organization        
Eligibility Requirements
For All State & Locality Representatives

In order to become a State and or Locality Representative for Cushing’s Understanding Support and Help Organization (CUSH) you must request an application from CUSH and submit the completed application to Cushing’s Understanding Support & Help Organization P.O. Box 452 Florence, AL. 35631-0452 The submitted application will be forwarded to all Board Members by email, fax or postal mail, for evaluation.  Once all Board Members have evaluated the submitted application there will be a meeting of all Executive Board Members and at least one member from the general Board of Directors for voting.  The meeting for voting can either be in person, online or via phone. In order for the applicant to become a State or Locality Representative it must be by unanimous vote.

The following criteria must be met by an applicant in order to have a vote.
If any of the following criteria is not met then the applicant is not eligible to become a
State and or Locality Representative for CUSH.

1.  Applicant must be a member of CUSH, in good standing.

2.  Applicant must live within the State and or Locality, or within 100 miles of the state or    
     Locality represented.

3. Applicant must be willing to host and organize a meeting for CUSH at least once per
     year.

4.  Applicant must have a confirmed Cushing’s diagnosis or have an immediate family member 
     (spouse, child, parent, sibling) with a confirmed Cushing’s diagnosis.
     Those having Cushing’s previously and are now cured are considered confirmed. 

5.  Applicant must agree to all conditions, agreements, guidelines, and expected duties
     on the state and or locality representative application.       

All applicants should complete an application in full and submit it to CUSH. The application includes some specific information that is required. You can print the application or request an application from the State Representative page on this website.
